Two researchers interviewed all customers of a number of community pharmacies asking for pregabalin products during the observational period (November 2016–January 2017). Pregabalin is a widely prescribed medication used to manage a range of conditions including neuropathic pain, epilepsy, fibromyalgia, generalized anxiety… The NMURx survey methodology is useful to identify under-documented use and misuse of medication and can detect changes in pregabalin price usa trends of substance use and misuse; also, it permits to make comparisons among different countries.

Research suggests that gabapentinoids alone rarely cause death, but clinically-relevant doses can be fatal, possibly owing to their reducing of tolerance to opioids​11​. “This explains why some patients may suffer from respiratory depression and potential deaths if they have lost tolerance to opioids,” says Guirguis. Pregabalin was reclassified after a public consultation and advice from the Advisory Council on the Misuse of Drugs, which was concerned about the risks of “illegal diversion and medicinal misuse”. It followed a marked increase in the number of deaths linked to gabapentinoids, which can cause euphoria, sedation and dissociation, making them sought after as a recreational drug. Amira Guirguis, MPharm programme director at Swansea University and an expert in novel psychoactive substances, adds that the potency of pregabalin might explain prescribers’ preference for it over gabapentin. “Across various studies, pregabalin consistently demonstrates a greater potency than gabapentin​3​.

Greater transparency in the reporting of outcomes is advocated; independent and publicly funded trials assessing the effects of pregabalin in neuropathic pain should be encouraged. Allowing researchers access to full CSRs of pregabalin trials should be a priority for drug companies and regulators. Experts’ ratings in the 5 separate dimensions are shown in the (Supplementary Figures 4–8). Regarding overall harm, traditional drugs of abuse, i.e., cocaine (including “crack”), methamphetamine, heroin, and alcohol were ranked as being most harmful. The NPS, i.e., cathinones and synthetic cannabinoids, had subordinate positions in the top harm-level group.

Two studies have suggested that there is a slightly increased chance of birth defects in the baby following use of pregabalin in early pregnancy. Other studies do not agree with this and, overall, it is very clear that most pregnant women using pregabalin will have a healthy baby. Pregabalin (Alzain®, Axalid®, Lyrica®) is used to treat epilepsy, some types of nerve pain, and anxiety. The increasing number of drug-related deaths may in part be addressed by ensuring that pregabalin is being prescribed appropriately. Pregabalin misusers achieve euphoric effects by taking large quantities, ranging from 200mg to 5g as a single dose (maximum licensed dose is 600mg daily in two or three divided doses); its pharmacokinetics make it relatively more dangerous than gabapentin in high doses​12​. Pregabalin is recommended by the National Institute for Health and Care Excellence (NICE) as one of the first-line options for neuropathic pain (except for trigeminal neuralgia) in adults, along with gabapentin, amitriptyline and duloxetine​5​.
